Approve and put into effect from December 1, 2014 the attached Guidelines on the military (ship) economy in the Armed Forces of the Russian Federation.

On December 1, 2014, the Guidelines for the military (ship) economy in the Armed Forces of the Russian Federation are put into effect.

The latter includes the material and technical base, weapons, military and special equipment and other material assets of formations (military units), land plots.

Economic activity is aimed at creating conditions for organizing and conducting combat training and educational work; bath and laundry service; control of the operation of the material and technical base; safe use of objects of state technical supervision; measures of environmental safety, fire protection, as well as environmental protection, etc.

An integral part of the military economy and its basis is the company. It is intended to provide combat training, educational work and meet the material, housing, domestic and cultural needs of soldiers (sailors) and sergeants (foremen).

Specific measures for managing the economy and features of their implementation are provided.

b) family members (spouses, parents, children under the age of 18 and children under the age of 23 studying in educational institutions full-time) Heroes of the Russian Federation, Heroes of the Soviet Union, full holders of the Order of Glory from among the military personnel and pensioners of the Ministry defense;

c) spouses and parents of the deceased (deceased) Heroes of the Russian Federation, Heroes of the Soviet Union, full holders of the Order of Glory from among the military personnel and pensioners of the Ministry of Defense;

d) widows (widowers) of Heroes of Socialist Labor or full holders of the Order of Labor Glory who have not remarried (regardless of the date of death (death) of the Hero of Socialist Labor or full holder of the Order of Labor Glory);

e) military personnel undergoing military service under a contract (with the exception of military personnel undergoing military service under a contract in formations and military units of constant readiness in positions to be recruited by soldiers, sailors, sergeants and foremen, and who entered military service under a contract after January 1, 2004 , as well as cadets of military educational institutions of vocational education), who performed tasks in the conditions of an armed conflict of a non-international nature in the Chechen Republic and in the territories of the North Caucasus immediately adjacent to it, classified as an armed conflict zone (from December 1994 to December 1996 ), as well as participating in counter-terrorist operations and ensuring law and order and public security on the territory of the North Caucasus region of the Russian Federation;

g) other persons in accordance with the legislation of the Russian Federation.

4. In accordance with the legislation of the Russian Federation, the privileged right to receive vouchers to health resorts of the Ministry of Defense is enjoyed by:

a) military personnel and citizens dismissed from military service upon reaching the age limit for military service, for health reasons or in connection with organizational and staff measures - participants in the war (Civil War, Soviet-Polish War, War with Finland, World War II , wars with Japan);

b) combat veterans from among the military personnel and pensioners of the Ministry of Defense;

c) persons awarded the badge "Inhabitant of besieged Leningrad";

d) persons who worked during the Great Patriotic War at the objects of air defense, local air defense, the construction of defensive structures, naval bases, airfields and other military facilities within the rear borders of the operating fronts, operational zones of the operating fleets, on the front-line sections of railways and roads ;

e) family members of the fallen (deceased) disabled veterans of the Great Patriotic War and combat invalids, participants of the Great Patriotic War and combat veterans;

f) family members of servicemen who died in the line of military service;

g) members of the families of military personnel who died in captivity, recognized as missing in the established order in the areas of hostilities, from the time of the exclusion of these military personnel from the lists of military units;

h) other persons in accordance with the legislation of the Russian Federation.

5. Servicemen undergoing military service under a contract (with the exception of servicemen undergoing military service under a contract in formations and military units of constant readiness in positions to be recruited by soldiers, sailors, sergeants and foremen, and who entered military service under a contract after January 1, 2004 as well as cadets of military educational institutions of vocational education) who were injured (wounded, traumatized, shell-shocked) or ill in the performance of military service duties, after hospital treatment, they have the right to receive extraordinary vouchers to sanatorium-and-spa institutions of the Ministry of Defense.

6. Target allocation of vouchers for hydronauts from among the military personnel specified in this Procedure to sanatorium-and-spa institutions of the Ministry of Defense is carried out at the place of their attachment to medical support.

7. In accordance with the legislation of the Russian Federation, vouchers to sanatoriums and resorts are provided with payment:

a) servicemen undergoing military service under a contract (with the exception of servicemen undergoing military service under a contract in formations and military units of constant readiness in positions to be recruited by soldiers, sailors, sergeants and foremen, and who entered military service under a contract after January 1, 2004 as well as cadets of military educational institutions of vocational education), officers dismissed from military service upon reaching the age limit for military service, for health reasons or in connection with organizational and staff activities, the total duration of military service of which in a preferential calculation is 20 years or more, and with a total duration of military service of 25 years or more, regardless of the reason for dismissal, - 25 percent, and for family members of military personnel - citizens and dependents of the military personnel specified in this subparagraph, and living together with them - 50 percent of the cost of pu tevki;

b) warrant officers and midshipmen dismissed from military service upon reaching the age limit for military service, for health reasons or in connection with organizational and staff measures, the total duration of military service of which is 20 years or more - 25 percent of the cost of the voucher;

c) military servicemen undergoing military service under a contract in formations and military units of constant readiness in positions to be recruited by soldiers, sailors, sergeants and foremen, and who entered military service under a contract after January 1, 2004, as well as military personnel undergoing military service on conscription, and cadets of military educational institutions of vocational education in accordance with the conclusion of the military medical commission to continue hospital treatment - free of charge;

d) Heroes of the Russian Federation, Heroes of the Soviet Union, full cavaliers of the Order of Glory (hereinafter referred to as Heroes) from among the military and pensioners of the Ministry of Defense of the Russian Federation:

those who use the benefits of health resort services - free of charge, and their family members (spouses, parents, children under the age of 18 and children under the age of 23 studying in educational institutions full-time) - 25 percent of the cost of the voucher;

those who receive monthly cash payments - 100 percent of the cost of the voucher, and for members of their families:

spouses, dependents of the military personnel specified in this Procedure and living together with them, children under the age of 18 and children under the age of 23 studying in educational institutions in full-time education, Heroes from among the military personnel undergoing military service under a contract (with the exception of servicemen undergoing military service under a contract in formations and military units of constant readiness in positions to be recruited by soldiers, sailors, sergeants and foremen, and who entered military service under a contract after January 1, 2004, as well as cadets military educational institutions of vocational education), - 50 percent of the cost of the voucher;

spouses, dependents of the military personnel specified in this Procedure and living together with them, children under the age of 18 and children under the age of 23 studying in educational institutions on a full-time basis, Heroes from among the officers dismissed from military service upon reaching the age limit for military service, health status or in connection with organizational and staff measures, the total duration of military service of which in a preferential calculation is 20 years or more, and with a total duration of military service of 25 years or more, regardless of grounds for dismissal - 50 percent of the cost of the voucher;

e) widows (widowers) and parents of deceased (deceased) Heroes from among the military personnel and pensioners of the Ministry of Defense, who use benefits for sanatorium and resort services - 25 percent, and who receive monthly cash payments - 100 percent of the cost of the tour;

f) Heroes of Socialist Labor, full cavaliers of the Order of Labor Glory from among the pensioners of the Ministry of Defense, who enjoy the benefit of sanatorium and resort services - free of charge, and who receive monthly cash payments - 100 percent of the cost of the voucher;

g) family members of Heroes of Socialist Labor, full cavaliers of the Order of Labor Glory from among the pensioners of the Ministry of Defense specified in this Procedure - 50 percent of the cost of the voucher;

At the same time, the sanatorium-resort institution on the ticket (in the section for special notes) for the persons specified in this clause makes the appropriate mark: "Hero of the Russian Federation, Hero of the Soviet Union and full holder of the Order of Glory", "Hero of Socialist Labor and full holder of the Order Labor Glory", "Family member of the Hero of the Russian Federation, Hero of the Soviet Union and full holder of the Order of Glory", "Widow (widower), father, mother of the Hero of the Russian Federation, Hero of the Soviet Union and full holder of the Order of Glory", as well as the series and number certificates;

h) children of military personnel specified in, and this Procedure, under the age of 18, sent in the prescribed manner to the Central Military Children's Sanatorium of the Ministry of Defense - free of charge;

i) spouses and parents of military personnel who died (missing) while performing tasks in the North Caucasus region of the Russian Federation in connection with the performance of official duties after August 1, 1999, as well as spouses and parents of military personnel who died in the line of military service on the nuclear submarine "Kursk", - free of charge;

j) servicemen undergoing military service under a contract in formations and military units of constant readiness in positions to be recruited by soldiers, sailors, sergeants and foremen, and who entered military service under a contract after January 1, 2004, in accordance with the conclusion of the military medical commissions (except for cases of sending them to sanatorium-and-spa treatment to continue hospital treatment) - 100 percent of the cost of the voucher. At the same time, the sanatorium-resort institution makes a note on the voucher: "With payment of 100 percent of the cost of the voucher for military personnel in accordance with the Decree of the Government of the Russian Federation of August 20, 2004 No. 423";

k) persons of the civilian personnel of the Armed Forces on the terms determined by the agreement between the trade unions of the civilian personnel of the Armed Forces and the Ministry of Defense, concluded in the prescribed manner. At the same time, the sanatorium-resort institution makes a mark on the vouchers "For civilian personnel of the Armed Forces of the Russian Federation."

information stands (other technical means of a similar purpose) in the premises occupied by the relevant structures (including military units) of the Ministry of Defense, and other places designated for these purposes for promptly informing citizens with information.

Postal addresses, the actual location of the Department and sanatorium-and-spa institutions are posted on the official website of the Ministry of Defense on the Internet, as well as in each military administration, association administration, formation administration, military unit, organization of the Armed Forces on special boards, on which posted the daily routine, time regulations.

14. The direction of citizens to sanatorium-resort institutions is carried out by decision of the Department on the basis of a written application of a citizen according to the recommended sample in accordance with this Procedure.

At the same time, the citizens specified in, (with the exception of family members of officers dismissed from military service), and in this Procedure, send applications to the Department no later than thirty calendar days before the planned day of arrival at the corresponding sanatorium and resort institution.

e) conscripted military personnel, cadets of military educational institutions of vocational education - military ID, vacation ticket, food certificate, health resort card in the form No. 072 / y-04;

f) patients transferred to sanatoriums to continue hospital treatment - a passport (identity card or military ID), the conclusion of the military medical commission of the hospital, an extract from the medical history and a food certificate (for conscripted military personnel);

g) spouses and parents of servicemen who died (missing in action) while performing tasks on the territory of the North Caucasus region of the Russian Federation in connection with the performance of official duties after August 1, 1999, as well as spouses and parents of servicemen who died in the line of military service on the Kursk nuclear submarine - a passport, a certificate certifying the relationship to the deceased soldier, issued by the military commissariat according to the recommended model in accordance with this Procedure, and a sanatorium card in the form No. 072 / y-04 or a health certificate when referral to a holiday home, compulsory medical insurance policy;

h) family members of military personnel who have lost their breadwinner, parents who have reached retirement age, and disabled parents of senior and senior officers who died (deceased) during their military service, as well as senior and senior officers who died (deceased) after being discharged from military service upon reaching the age limit for military service, health status or in connection with organizational and staffing events that had a total duration of military service of 20 years or more - a passport (children under 14 years old - a birth certificate), a certificate certifying a related relation to a serviceman who died (deceased) during military service under a contract, issued by the military commissariat according to the recommended model in accordance with this Procedure, a sanatorium-resort card in the form No. 072 / y-04 (for children - a sanatorium-resort card in the form 076 / y-04) or a certificate of health when sent to a holiday home, recreation center, compulsory medical insurance policy . In addition, parents - a pension certificate (certificate of a medical and social examination on the establishment of disability), a certificate issued by the military commissariat stating that they were dependent on a military man, according to the recommended sample in accordance with this Procedure, children under 14 years of age - analysis for enterobiasis , the conclusion of a dermatologist about the absence of contagious skin diseases, a certificate from a pediatrician or epidemiologist about the absence of contact of the child with infectious patients at the place of residence, in kindergarten or school.

i) persons of civilian personnel of the Armed Forces - a passport, a certificate of employment confirming their work in a military command body, military unit, institution (organization) of the Armed Forces, indicating the source of financing of labor costs (from the federal budget allocated to the Ministry defense, or income of institutions (organizations) of the Armed Forces received from entrepreneurial and other income-generating activities) according to the recommended sample in accordance with this Procedure, a sanatorium-and-spa card in the form No. 072 / y-04 or a health certificate when sent to a rest home recreation center, compulsory medical insurance policy.

21. Citizens are admitted to sanatorium-and-spa institutions and discharged from them within the time specified in the vouchers.

22. When discharged from a sanatorium, a citizen is issued a discharge summary and a voucher for a voucher, and from a rest home and recreation center - a voucher for a voucher.

For citizens who arrived with food certificates, the date of their exclusion from allowance is indicated based on the actual time spent in the sanatorium.

23. In case of early departure of citizens from a sanatorium-resort institution, a refund for unused days of the voucher is made by sanatorium-resort institutions on the basis of a written application of a citizen in the following cases:

a) recall from vacation in the prescribed manner;

b) sudden serious illness (death) of family members and close relatives: wife, husband, children, father, mother, brothers, sisters;

c) a natural disaster that has befallen a family or close relative;

d) transfer for health reasons to a medical institution (hospital).

In these cases, the persons specified in this Procedure do not lose the right to social guarantees for preferential sanatorium-resort treatment and recreation in sanatorium-resort institutions in the current year.

24. The term of treatment of a citizen in a sanatorium-resort institution, if there are medical indications, may be extended by the head of the sanatorium-resort institution only by order of the medical commission of the sanatorium for a period of not more than 10 days. At the same time, payment for the days of extension to the patient is made on the conditions under which he was admitted to the sanatorium-resort institution.

In case of extension of the period of treatment for medical reasons, one of the spouses who are jointly staying in a sanatorium-resort institution, the other spouse, at his request, is allowed to extend the stay in a sanatorium-resort institution (after the expiration of the voucher) with payment for the days of residence (extension) at the rate of full ticket price.

25. In order to make decisions on extending the period of treatment in a sanatorium-resort institution in the presence of medical contraindications or the need to transfer citizens requiring inpatient treatment to medical institutions, a medical commission is created in each sanatorium-resort institution. She is appointed by order of the head of the sanatorium and resort institution, consisting of the chairman, at least three members of the commission from among the doctors and the secretary. The chairman of the medical commission of the sanatorium-resort institution is the deputy head of the sanatorium-resort institution for the medical part or the leading therapist of the sanatorium-resort institution.

26. About all cases of sending citizens with contraindications to a sanatorium-resort institution, the head of the sanatorium-resort institution keeps a record of them and reports them in the prescribed form to the medical institution that issued the sanatorium-resort card.

Citizens arriving at a sanatorium-resort institution with contraindications to sanatorium-resort treatment, after the conclusion of the medical commission, are subject to return to their place of residence with the allocation of an accompanying person, if necessary.

V. Provision of vouchers to sanatorium-and-spa institutions for the continuation of hospital treatment

28. Until June 1 of the current year, the Main Military Medical Directorate of the Ministry of Defense of the Russian Federation (hereinafter referred to as the Main Military Medical Directorate) provides the Department with the need for hospital beds to continue hospital treatment in sanatoriums and resorts for the next calendar year.

Until December 1 of the current year, the Department sends information to the Main Military Medical Directorate on the number of places in sanatorium-and-spa institutions for the continuation of hospital treatment for the next calendar year.

29. In order to continue hospital treatment, in accordance with the conclusion of the hospital military medical commission, citizens from the list of persons indicated in this Procedure are sent to sanatorium and resort institutions.

30. Vouchers to sanatorium-resort institutions for the continuation of hospital treatment are allocated in accordance with the established procedure on applications sent by hospitals to the Department.

To resolve the issue of transferring citizens from hospitals to sanatorium-resort institutions, an extract from the medical history and the conclusion of the hospital military medical commission are sent along with the application.

Upon positive consideration of the application for a ticket (in the section for

special marks) the sanatorium-resort institution makes a record:

31. The list of diseases for which it is allowed to send patients to sanatorium-and-spa institutions for the continuation of hospital treatment is developed by the Main Military Medical Directorate in agreement with the Department.

32. In the absence of sanatorium-resort institutions of the required profile in the Ministry of Defense, sanatorium-resort treatment of citizens from the list of persons specified in this Procedure is carried out on a contractual basis in specialized sanatorium-resort institutions of other federal executive bodies or other organizations that have the appropriate license for medical activity.

33. As part of the implementation of the legislation of the Russian Federation in terms of aftercare (rehabilitation) of patients in a sanatorium, the heads of military hospitals are allowed to conclude agreements with regional offices of the Social Insurance Fund of the Russian Federation on the allocation of vouchers for aftercare of patients from among working insured citizens (pensioners of the Ministry of Defense, family members military personnel and pensioners of the Ministry of Defense, as well as civilian personnel of the Armed Forces (including federal civil servants).

Vouchers for aftercare of the citizens specified in this paragraph, immediately after inpatient treatment are provided free of charge at the expense of compulsory social insurance.

Referral for aftercare immediately after inpatient treatment to specialized sanatoriums (departments) on vouchers allocated by the Social Insurance Fund of the Russian Federation is carried out in the manner prescribed by the legislation of the Russian Federation.

This Order shall enter into force 10 days after the date of its official publication.

Document overview

A new procedure for sanatorium and resort provision in the Armed Forces of the Russian Federation has been approved.

The main changes are as follows.

It has been clarified who is sent to the sanatorium and resort institutions of the Russian Defense Ministry on preferential terms (once a year). This includes dependents of retired officers who live with them. We are talking about officers whose total length of service on a preferential basis is 20 years or more, dismissed for certain reasons. This is the achievement of the age limit, the state of health or organizational and staff activities. With a total length of service of 25 years or more, the basis for dismissal does not matter.

Previously, the plan for using the bed capacity of the relevant institutions was transferred to the Main Military Medical Directorate of the Russian Ministry of Defense. It distributed tickets. Now the Department for Sanatorium and Resort Provision of the Ministry is engaged in this.

Citizens are sent to the appropriate institutions by decision of the named Department. Before, sanatorium-selection commissions were formed.

It is fixed how citizens should be informed about the procedure for sanatorium and resort provision.

It is envisaged that a register of persons sent to sanatorium-and-spa institutions is maintained.

Now the citizen is sent a notification about the provision of a ticket. She herself is issued, paid for and issued in a sanatorium-resort institution on the day of its admission. Previously, a ticket was issued, certified by the seal of the sanatorium-selection commission.

The composition of the documents presented upon admission to the institution has been revised. So, some citizens require a compulsory health insurance policy. For children under 14 years old, an analysis for enterobiosis, a conclusion that there are no contagious skin diseases and a certificate that the child has not been in contact with infectious patients are needed.

I. General provisions

1. In the sanatorium, rest houses, recreation centers of the Ministry of Defense of the Russian Federation (hereinafter referred to as sanatorium-resort organizations), on the basis of medical indications and in the absence of contraindications during the holidays, but not more than once a year, on preferential terms, are sent:

b) officers dismissed from military service upon reaching the age limit for military service, for health reasons or in connection with organizational and staff measures, the total duration of military service of which in a preferential calculation is 20 years or more, and with a total duration of military service of 25 years or more, regardless of the reason for dismissal;

c) warrant officers and midshipmen dismissed from military service upon reaching the age limit for military service, for health reasons or in connection with organizational and staffing measures, the total duration of military service of which is 20 years or more;

d) family members of military personnel and officers discharged from military service specified in subparagraphs "a" and "b" of this paragraph (spouse (husband) and children under the age of 18, as well as older than this age, who became disabled before they reached the age of 18 years old, children under the age of 23 years old, studying in educational institutions full-time), as well as persons who are dependents of the military personnel specified in subparagraphs "a" and "b" of this paragraph, and living together with them. At the same time, children under 18 years of age are sent, if there are medical indications, to children's sanatoriums. Children under the age of 18 who do not need special treatment are sent to sanatorium-and-spa organizations, which provide for family vacations, on vouchers together with their parents (parent);

e) family members of military personnel who have lost their breadwinner, parents who have reached retirement age, and disabled parents of senior and senior officers who died (deceased) during their military service, as well as senior and senior officers who died (deceased) after being discharged from the military service upon reaching the age limit for military service, health status or in connection with organizational and staffing measures, which had a total duration of military service of 20 years or more, who, during the life of a serviceman, enjoyed the right to sanatorium-and-spa treatment;

f) widows (widowers) of military personnel who perished (deceased) during the period of military service under a contract or after being discharged from military service upon reaching the age limit for military service, health status or in connection with organizational and staffing activities that had a total duration of military service for 20 years or more, who, during the life of a serviceman, enjoyed the right to sanatorium-and-spa treatment - until remarriage;

g) spouses and parents of servicemen who died (missing) while performing tasks in the North Caucasus region of the Russian Federation in connection with the performance of official duties after August 1, 1999;

h) spouses and parents of servicemen who died in the line of military service on the nuclear submarine Kursk;

i) Heroes of the Russian Federation, Heroes of the Soviet Union, Heroes of Socialist Labor, full cavaliers of the Orders of Glory and Labor Glory from among the military personnel and pensioners of the Ministry of Defense;

j) family members (spouses, parents, children under the age of 18 and children under the age of 23 studying in educational institutions full-time) Heroes of the Russian Federation, Heroes of the Soviet Union, full holders of the Order of Glory from among the military personnel and pensioners of the Ministry defense;

k) spouses and parents of the deceased (deceased) Heroes of the Russian Federation, Heroes of the Soviet Union, full holders of the Order of Glory from among the military personnel and pensioners of the Ministry of Defense;

l) persons of the civilian personnel of the Armed Forces (in the event that an industry agreement between the trade unions of the civilian personnel of the Armed Forces and the Ministry of Defense establishes such obligations for the relevant period in the manner and on the terms determined by this agreement).

2. In accordance with the conclusion of the military medical commission, the following are sent to the sanatorium for medical rehabilitation after inpatient treatment:

a) military personnel undergoing military service under a contract and members of their families (with the exception of family members of military personnel serving under a contract in formations and military units of constant readiness in positions to be recruited by soldiers, sailors, sergeants and foremen, and who entered the military contract service after January 1, 2004, as well as cadets of military educational institutions of vocational education);

b) pensioners of the Ministry of Defense from among:

officers dismissed from military service upon reaching the age limit for military service, for health reasons or in connection with organizational and staff measures, the total duration of military service of which in a preferential calculation is 20 years or more, and with a total duration of military service of 25 years and more, regardless of the grounds for dismissal, and members of their families;

ensigns and midshipmen dismissed from military service upon reaching the age limit for military service, for health reasons or in connection with organizational and staff measures, the total duration of military service of which is 20 years or more;

d) cadets of military educational institutions of professional education;

e) conscripted military personnel.

II. Social guarantees for persons entitled to sanatorium treatment and recreation.

d) widows (widowers) of Heroes of Socialist Labor or full cavaliers of the Order of Labor Glory from among the military personnel and pensioners of the Ministry of Defense who have not remarried (regardless of the date of death (death) of the Hero of Socialist Labor or full cavalier of the Order of Labor Glory);

g) other persons in accordance with the legislation of the Russian Federation.

b) combat veterans from among the military personnel and pensioners of the Ministry of Defense;

c) persons awarded the badge "Inhabitant of besieged Leningrad", from among the military personnel and pensioners of the Ministry of Defense;

e) family members of the fallen (deceased) disabled veterans of the Great Patriotic War and invalids of military operations, participants of the Great Patriotic War and combat veterans from among the military personnel and pensioners of the Ministry of Defense;

f) family members of servicemen who died in the line of military service;

h) other persons in accordance with the legislation of the Russian Federation.

5. Servicemen undergoing military service under a contract (with the exception of servicemen undergoing military service under a contract in formations and military units of constant readiness in positions to be recruited by soldiers, sailors, sergeants and foremen, and who entered military service under a contract after January 1, 2004 as well as cadets of military educational institutions of vocational education) who have been injured (wounded, traumatized, contused) or ill in the performance of military service duties, after treatment in a hospital, have the right to receive extraordinary vouchers to sanatorium organizations of the Ministry of Defense.

6. Target allocation of vouchers for hydronauts from among the military personnel specified in this Procedure to sanatorium and resort organizations of the Ministry of Defense is carried out at the place of their attachment to medical support.

7. In accordance with the legislation of the Russian Federation, vouchers to sanatorium organizations are provided with payment:

a) servicemen undergoing military service under a contract (with the exception of servicemen undergoing military service under a contract in formations and military units of constant readiness in positions to be recruited by soldiers, sailors, sergeants and foremen, and who entered military service under a contract after January 1, 2004 d., as well as cadets of military educational organizations of higher education, military professional educational organizations), family members of military personnel - 100 percent of the cost of the voucher; officers dismissed from military service upon reaching the age limit for military service, for health reasons or in connection with organizational and staff measures, the total duration of military service of which in a preferential calculation is 20 years or more, and with a total duration of military service of 25 years and more, regardless of the reason for dismissal - 25 percent, to members of their families and persons who are dependent on the military personnel specified in this paragraph and living together with them - 50 percent of the cost of the voucher;

c) servicemen undergoing military service under a contract in formations and military units of constant readiness in positions to be recruited by soldiers, sailors, sergeants and foremen, and who entered military service under a contract after January 1, 2004, as well as servicemen undergoing military service on conscription, and cadets of military educational institutions of vocational education in accordance with the conclusion of the military medical commission for medical rehabilitation after treatment in a hospital - free of charge;

those who receive monthly cash payments - 100 percent of the cost of the voucher, and for members of their families:

spouses, dependents of the military personnel specified in this Procedure and living together with them, children under the age of 18 and children under the age of 23 studying in educational institutions on a full-time basis, Heroes from among the officers dismissed from military service upon reaching the age limit for military service, health status or in connection with organizational and staff measures, the total duration of military service of which in a preferential calculation is 20 years or more, and with a total duration of military service of 25 years or more, regardless of grounds for dismissal - 50 percent of the cost of the voucher;

At the same time, the sanatorium-and-spa organizations on the ticket (in the section for special notes) for the persons specified in this paragraph, the corresponding mark is made: "Hero of the Russian Federation, Hero of the Soviet Union and full cavalier of the Order of Glory", "Hero of Socialist Labor and full cavalier of the Order Labor Glory", "Family member of the Hero of the Russian Federation, Hero of the Soviet Union and full holder of the Order of Glory", "Widow (widower), father, mother of the Hero of the Russian Federation, Hero of the Soviet Union and full holder of the Order of Glory", as well as the series and number certificates;

h) children of the military personnel specified in and this Procedure, under the age of 18, sent in the prescribed manner to the sanatorium and resort organizations of the Ministry of Defense, determined by the Main Military Medical Directorate of the Ministry of Defense of the Russian Federation (hereinafter referred to as the Main Directorate), - free of charge;

j) servicemen undergoing military service under a contract in formations and military units of constant readiness in positions to be recruited by soldiers, sailors, sergeants and foremen, and who entered military service under a contract after January 1, 2004, in accordance with the conclusion of the military medical commissions (except for cases of sending them to sanatorium-and-spa treatment for medical rehabilitation after inpatient treatment) - 100 percent of the cost of the voucher. At the same time, the sanatorium-resort organization makes a note on the voucher: "With payment of 100 percent of the cost of the voucher for military personnel in accordance with the Decree of the Government of the Russian Federation of August 20, 2004 N 423";

k) persons of the civilian personnel of the Armed Forces on the terms determined by the agreement between the trade unions of the civilian personnel of the Armed Forces and the Ministry of Defense, concluded in the prescribed manner. At the same time, the sanatorium-resort organization makes a mark on the vouchers "For civilian personnel of the Armed Forces of the Russian Federation."

In the sanatorium, rest homes, recreation centers of the Ministry of Defense of the Russian Federation1 (hereinafter referred to as sanatorium-and-spa institutions), on the basis of medical indications and in the absence of contraindications during the holidays, but not more than once a year, on preferential terms, are sent:

C) warrant officers and midshipmen dismissed from military service upon reaching the age limit for military service, for health reasons or in connection with organizational and staff measures, the total duration of military service of which is 20 years or more;

D) family members of military personnel and officers discharged from military service specified in subparagraphs "a" and "b" of this paragraph (spouse (husband) and children under the age of 18, as well as older than this age, who became disabled before they reached the age 18 years old, children under the age of 23 years old, studying in educational institutions full-time), as well as persons who are dependents of the military personnel specified in subparagraphs "a" and "b" of this paragraph, and living together with them. At the same time, children under 18 years of age are sent, if there are medical indications, to children's sanatoriums; children under the age of 18 who do not need special treatment are sent to sanatorium-and-spa institutions that provide for family vacations, on vouchers together with their parents (parent);

E) family members of military personnel who have lost their breadwinner, parents who have reached retirement age, and disabled parents of senior and senior officers who died (deceased) during their military service, as well as senior and senior officers who died (deceased) after being discharged from the military service upon reaching the age limit for military service, health status or in connection with organizational and staffing measures, which had a total duration of military service of 20 years or more, who, during the life of a serviceman, enjoyed the right to sanatorium-and-spa treatment;

E) widows (widowers) of military personnel who died (deceased) during the period of military service under a contract or after being discharged from military service upon reaching the age limit for military service, health status or in connection with organizational and staff activities that had a total duration of military service for 20 years or more, who, during the life of a serviceman, enjoyed the right to sanatorium-and-spa treatment - until remarriage;

G) spouses and parents of military personnel who died (missing in action) while performing tasks on the territory of the North Caucasus region of the Russian Federation in connection with the performance of official duties after August 1, 1999;

3) spouses and parents of military personnel who died in the line of military service on the nuclear submarine Kursk;

I) Heroes of the Russian Federation, Heroes of the Soviet Union, Heroes of Socialist Labor, full cavaliers of the Orders of Glory and Labor Glory from among the military personnel and pensioners of the Ministry of Defense;

J) family members (spouses, parents, children under the age of 18 and children under the age of 23 studying in educational institutions full-time) Heroes of the Russian Federation, Heroes of the Soviet Union, full holders of the Order of Glory from among the military personnel and pensioners of the Ministry defense;

L) spouses and parents of the deceased (deceased) Heroes of the Russian Federation, Heroes of the Soviet Union, full holders of the Order of Glory from among the military personnel and pensioners of the Ministry of Defense;

L) persons of the civilian personnel of the Armed Forces (in the event that an industry agreement between the trade unions of the civilian personnel of the Armed Forces and the Ministry of Defense establishes such obligations for the relevant period in the manner and on the terms determined by this agreement).

2. In accordance with the conclusion of the hospital military medical commission, the following are sent to the sanatorium to continue hospital treatment:

A) military personnel undergoing military service under a contract, and members of their families (with the exception of family members of military personnel serving under a contract in formations and military units of constant readiness in positions to be recruited by soldiers, sailors, sergeants and foremen, and entered the military contract service after January 1, 2004, as well as cadets of military educational institutions of vocational education);

B) pensioners of the Ministry of Defense from among:

Officers dismissed from military service upon reaching the age limit for military service, for health reasons or in connection with organizational and staff measures, the total duration of military service of which in a preferential calculation is 20 years or more, and with a total duration of military service of 25 years and more, regardless of the grounds for dismissal, and members of their families;

Ensigns and midshipmen dismissed from military service upon reaching the age limit for military service, for health reasons or in connection with organizational and staff measures, the total duration of military service of which is 20 years or more;

C) servicemen undergoing military service under a contract in formations and military units of constant readiness in positions to be recruited by soldiers, sailors, sergeants and foremen, and who entered military service under a contract after January 1, 2004;

D) cadets of military educational institutions of professional education;

D) conscripted military personnel.

II. Social guarantees for persons entitled to sanatorium treatment and recreation. Ticket price

3. In accordance with the legislation of the Russian Federation, the right to receive priority vouchers to sanatorium-and-spa institutions is enjoyed by:

A) Heroes of the Russian Federation, Heroes of the Soviet Union, Heroes of Socialist Labor, full cavaliers of the Orders of Glory and Labor Glory from among military personnel and pensioners of the Ministry of Defense2;

B) family members (spouses, parents, children under the age of 18 and children under the age of 23 studying in educational institutions full-time) Heroes of the Russian Federation, Heroes of the Soviet Union, full holders of the Order of Glory from among the military personnel and pensioners of the Ministry defense3;

C) spouses and parents of the deceased (deceased) Heroes of the Russian Federation, Heroes of the Soviet Union, full holders of the Order of Glory from among the military personnel and pensioners of the Ministry of Defense3;

D) widows (widowers) of Heroes of Socialist Labor or full cavaliers of the Order of Labor Glory who have not remarried (regardless of the date of death (death) of the Hero of Socialist Labor or full cavalier of the Order of Labor Glory) 4 ;

E) military personnel undergoing military service under a contract (with the exception of military personnel undergoing military service under a contract in formations and military units of constant readiness in positions to be recruited by soldiers, sailors, sergeants and foremen, and who entered military service under a contract after January 1, 2004 as well as cadets of military educational institutions of vocational education) who performed tasks in the conditions of an armed conflict of a non-international character in the Chechen Republic and in the territories of the North Caucasus immediately adjacent to it, classified as an armed conflict zone5 (from December 1994 to December 1996 ), as well as participating in counter-terrorist operations and ensuring law and order and public security on the territory of the North Caucasus region of the Russian Federation6;

E) persons specified in subparagraphs "a" - "c" and "m" of paragraph 1 of this Procedure, awarded with the badge "Honorary Donor of Russia"7;

G) other persons in accordance with the legislation of the Russian Federation.

4. In accordance with the legislation of the Russian Federation, the privileged right to receive vouchers to health resorts of the Ministry of Defense is enjoyed by:

A) military personnel and citizens dismissed from military service upon reaching the age limit for military service, for health reasons or in connection with organizational and staff measures - participants in the war8 (Civil War, Soviet-Polish War, War with Finland, World War II , wars with Japan)9;

B) combat veterans from among the military personnel and pensioners of the Ministry of Defense9;

C) persons awarded the badge "Inhabitant of besieged Leningrad"9;

D) persons who worked during the Great Patriotic War at the objects of air defense, local air defense, construction of defensive structures, naval bases, airfields and other military facilities within the rear borders of the active fronts, operational zones of the operating fleets, on the front-line sections of railways and roads ten ;

E) family members of the fallen (deceased) disabled veterans of the Great Patriotic War and combat invalids, participants of the Great Patriotic War and combat veterans10;

E) family members of servicemen who died in the line of military service10;

G) family members of servicemen who died in captivity, recognized as missing in the established order in the areas of hostilities, since the exclusion of these servicemen from the lists of military units 10 ;

H) other persons in accordance with the legislation of the Russian Federation.

5. Servicemen undergoing military service under a contract (with the exception of servicemen undergoing military service under a contract in formations and military units of constant readiness in positions to be recruited by soldiers, sailors, sergeants and foremen, and who entered military service under a contract after January 1, 2004 as well as cadets of military educational institutions of vocational education) who have been injured (wounded, traumatized, contused) or ill while performing military service duties, after hospital treatment, they have the right to receive extraordinary vouchers to sanatorium-and-spa institutions of the Ministry of Defense 11 .

6. Target allocation of vouchers for hydronauts from among the military personnel specified in subparagraph "a" of paragraph 1 of this Procedure to sanatorium-resort institutions of the Ministry of Defense is carried out at the place of their attachment to medical support.

7. In accordance with the legislation of the Russian Federation, vouchers to sanatoriums and resorts are provided with payment:

A) military personnel undergoing military service under a contract (with the exception of military personnel undergoing military service under a contract in formations and military units of constant readiness in positions to be recruited by soldiers, sailors, sergeants and foremen, and who entered military service under a contract after January 1, 2004 as well as cadets of military educational institutions of vocational education), officers dismissed from military service upon reaching the age limit for military service, for health reasons or in connection with organizational and staff activities, the total duration of military service of which in a preferential calculation is 20 years or more, and with a total duration of military service of 25 years or more, regardless of the reason for dismissal, - 25 percent, and for family members of military personnel - citizens and dependents of the military personnel specified in this subparagraph, and living together with them - 50 percent of the cost of pu tevki 12;

B) warrant officers and midshipmen dismissed from military service upon reaching the age limit for military service, for health reasons or in connection with organizational and staff measures, the total duration of military service of which is 20 years or more - 25 percent of the cost of the ticket 12;

C) military servicemen undergoing military service under a contract in formations and military units of constant readiness in positions to be recruited by soldiers, sailors, sergeants and foremen, and who entered military service under a contract after January 1, 2004, as well as military personnel undergoing military service on conscription, and for cadets of military educational institutions of vocational education in accordance with the conclusion of the military medical commission to continue hospital treatment - free of charge 13 ;

D) To the Heroes of the Russian Federation, Heroes of the Soviet Union, full holders of the Order of Glory (hereinafter referred to as the Heroes) from among the military personnel and pensioners of the Ministry of Defense of the Russian Federation:

Those who use the benefits of sanatorium and resort services - free of charge, and their family members (spouses, parents, children under the age of 18 and children under the age of 23 studying in educational institutions full-time) - 25 percent of the cost of the voucher 14;

Those who receive monthly cash payments - 100 percent of the cost of the voucher 14, and their family members:

Spouses, dependents of the military personnel specified in subparagraph "a" of paragraph 1 of this Procedure, and living together with them, children under the age of 18 and children under the age of 23 studying in educational institutions full-time education, Heroes from among the military personnel undergoing military service under a contract (with the exception of military personnel undergoing military service under a contract in formations and military units of constant readiness in positions to be recruited by soldiers, sailors, sergeants and foremen, and who entered military service under a contract after January 1, 2004 city, as well as cadets of military educational institutions of vocational education), - 50 percent of the cost of the voucher 15;

Spouses, dependents of the military personnel specified in subparagraph "b" of paragraph 1 of this Procedure, and living together with them, children under the age of 18 and children under the age of 23 studying in educational institutions full-time education, Heroes from the number of officers dismissed from military service upon reaching the age limit for military service, for health reasons or in connection with organizational and staff measures, the total duration of military service of which in a preferential calculation is 20 years or more, and with a total duration of military service of 25 years and more, regardless of the reason for dismissal, - 50 percent of the cost of the voucher 15;
e) widows (widowers) and parents of deceased (deceased) Heroes from among the military personnel and pensioners of the Ministry of Defense, who use benefits for sanatorium and resort services - 25 percent, and who receive monthly cash payments - 100 percent of the cost of the voucher 14;

E) Heroes of Socialist Labor, full cavaliers of the Order of Labor Glory from among the pensioners of the Ministry of Defense, who enjoy benefits for sanatorium and resort services - free of charge, and who receive monthly cash payments - 100 percent of the cost of the voucher 16;

G) family members of Heroes of Socialist Labor, full holders of the Order of Labor Glory from among the pensioners of the Ministry of Defense specified in subparagraph "b" of paragraph 1 of this Procedure - 50 percent of the cost of the voucher 16;

At the same time, the sanatorium-resort institution on the ticket (in the section for special marks) for the persons specified in subparagraphs "d" - "f" of this paragraph, makes the corresponding mark: "Hero of the Russian Federation, Hero of the Soviet Union and full cavalier of the Order of Glory", "Hero of Socialist Labor and full cavalier of the Order of Labor Glory", "Family member of the Hero of the Russian Federation, Hero of the Soviet Union and full cavalier of the Order of Glory", "Widow (widower), father, mother of the Hero of the Russian Federation, Hero of the Soviet Union and full cavalier of the Order of Glory ", as well as the series and number of the certificate;

3) children of military personnel specified in subparagraphs "a", "b" and "e" of paragraph 1 of this Procedure, under the age of 18, sent in the prescribed manner to the Central Military Children's Sanatorium of the Ministry of Defense - free of charge 17;

I) spouses and parents of military personnel who died (missing) while performing tasks in the North Caucasus region of the Russian Federation in connection with the performance of official duties after August 1, 1999, as well as spouses and parents of military personnel who died in the line of military service on the nuclear submarine "Kursk", - free of charge 18;

K) military personnel who are serving under a contract in formations and military units of constant readiness in positions to be recruited by soldiers, sailors, sergeants and foremen, and who entered military service under a contract after January 1, 2004, in accordance with the conclusion of the military medical commission (with the exception of cases when they are sent to sanatorium-and-spa treatment to continue hospital treatment) - 100 percent of the cost of the voucher 19 . At the same time, the sanatorium-resort institution makes a note on the voucher: "With payment of 100 percent of the cost of the voucher for military personnel in accordance with Decree of the Government of the Russian Federation of August 20, 2004 N 423";

K) to persons of civilian personnel of the Armed Forces on the terms determined by the agreement between the trade unions of the civilian personnel of the Armed Forces and the Ministry of Defense, concluded in the prescribed manner. At the same time, the sanatorium-resort institution makes a mark on the vouchers "For civilian personnel of the Armed Forces of the Russian Federation."

8. The persons specified in subparagraphs "a" and "b" of paragraph 2 of this Procedure, when they are sent to sanatorium-and-spa institutions to continue hospital treatment, in accordance with the conclusion of the hospital military medical commission, are additionally provided with free vouchers.

9. Persons who simultaneously have the right to receive the same social guarantee and compensation on several grounds are provided, at their choice, with a social guarantee and compensation on one basis, except for cases specifically provided for by the legislation of the Russian Federation.

10. Prices for vouchers to health resorts are determined in accordance with the established procedure.

III. Informing and sending citizens to sanatorium-resort institutions

11. Sanatorium and resort institutions annually by June 1 of the current year submit to the Department for Sanatorium and Resort Provision of the Ministry of Defense of the Russian Federation (hereinafter referred to as the Department) a plan for using the bed capacity of sanatorium and resort institutions for the next year, as well as information on the bed capacity by type numbers.

12. The Department develops and approves by November 1 of the current year a plan for the distribution of vouchers to sanatorium-resort institutions for the next calendar year and sends extracts from it to sanatorium-resort institutions.

13. The Department, together with other bodies of the military administration of the Ministry of Defense, organizes a system for informing citizens entitled to sanatorium treatment and recreation (hereinafter referred to as citizens) about the procedure for sanatorium and resort provision in accordance with the Federal Law of February 9, 2009. N 8-FZ "On ensuring access to information on the activities of state bodies and local governments" 20 through:

Mass media;

Official website of the Ministry of Defense on the Internet;

Information stands (other technical means of a similar purpose) in the premises occupied by the relevant structures (including military units) of the Ministry of Defense, and other places designated for these purposes for promptly informing citizens with information.

At the same time, the citizens specified in subparagraphs "a", "d" (with the exception of family members of officers dismissed from military service), "m" of paragraph 1 and in paragraph 2 of this Procedure, send applications to the Department no later than thirty calendar days before the planned day of arrival at the corresponding sanatorium-resort institution.

Citizens specified in subparagraphs "b" - "d" (with the exception of family members of military personnel), "e" - "l" of paragraph 1, send applications to the appropriate sanatorium-resort institution no later than sixty calendar days before the planned day of arrival in sanatorium-resort institution.

The application is accompanied by a certificate for obtaining a voucher in the form N 070 / y-04 21, issued in medical institutions.

15. Written applications from citizens are considered in accordance with Federal Law No. 59-FZ of May 2, 2006 "On the Procedure for Considering Appeals from Citizens of the Russian Federation" 22 . At the same time, applications from citizens specified in subparagraphs "a", "d" (only members of the families of military personnel) of paragraph 1 and paragraph 2 of this Procedure are considered for no more than ten calendar days.

16. The Department, on the basis of an application from a citizen in accordance with the approved plan for the distribution of vouchers and within the established time limits, sends a notification to the citizen about the provision of a voucher to a sanatorium-resort institution according to the recommended model in accordance with Appendix No. 2 to this Procedure.

17. The Department and sanatorium-resort institutions, in accordance with the established procedure, maintain a register of citizens sent to sanatorium-resort institutions of the Ministry of Defense of the Russian Federation (hereinafter referred to as the register), according to the recommended model in accordance with Appendix No. 3 to this Procedure. The processing of personal data of the categories of citizens specified in clauses 1 and 2 of this Procedure and included in the register is carried out in accordance with the Federal Law of July 27, 2006 N 152-FZ "On Personal Data" 23 and other regulatory legal acts of the Russian Federation that determine cases and features of personal data processing.

18. Medical examination of citizens of the Ministry of Defense specified in subparagraphs "b" and "c" of paragraph 1 of this Procedure, family members of officers discharged from military service (specified in paragraph "b" of paragraph 1 of this Procedure), as well as children entitled for sanatorium treatment, and obtaining a sanatorium card in the form N 072 / y-04 24 (for children - a sanatorium card in the form N 076 / y-04 25) or a certificate of health when sent to a rest home, as well as certificates for obtaining a voucher in the form N 070 / y-04 26 can be made at the place of residence in the relevant state (municipal) medical institutions or military medical institutions of the Ministry of Defense.

IV. Payment, issuance of vouchers, admission (discharge) to sanatorium-and-spa institutions

19. Registration, payment and issuance of a voucher to a sanatorium-resort institution (hereinafter referred to as a voucher) to a citizen is carried out in a sanatorium-resort institution on the day the citizen is received upon submission of a notice of the provision of a voucher issued by the Department.

20. Together with the notification of the voucher, citizens submit the following documents:

A) military personnel undergoing military service under a contract (with the exception of military personnel undergoing military service under a contract in formations and military units of constant readiness in positions to be recruited by soldiers, sailors, sergeants and foremen, and who entered military service under a contract after January 1, 2004 d., as well as cadets of military educational institutions of vocational education), - an identity card of a serviceman of the Russian Federation (hereinafter referred to as an identity card) (military ID) of the established form, a passport (if any), a vacation ticket and a sanatorium card in the form N 072 /y-04 27 or a health certificate when sent to a rest home, recreation center;

B) officers dismissed from military service upon reaching the age limit for military service, for health reasons or in connection with organizational and staff measures, the total duration of military service of which in a preferential calculation is 20 years or more, and with a total duration of military service of 25 years or more, regardless of the reason for dismissal, as well as warrant officers and midshipmen dismissed from military service upon reaching the age limit for military service, health reasons or in connection with organizational and staffing activities, the total duration of military service of which is 20 years or more , - a passport, a pension certificate, where in the section for special marks it should be indicated that he and his family members are entitled to social guarantees for the provision of medical care and sanatorium and resort treatment through the Ministry of Defense, a sanatorium and resort card in the form N 072 /u-04 27 or a health certificate when sent to the home ykha, a recreation center, a policy of compulsory medical insurance;

C) family members of servicemen and officers dismissed from military service - a passport (children under 14 years of age - a birth certificate), a compulsory medical insurance policy, a certificate of the established form certifying a family relationship to a serviceman (pensioner of the Ministry of Defense), issued by a military unit or military commissariat, and children aged 18 to 23 years, in addition, a certificate from the place of study confirming the fact of studying in an educational institution full-time; persons who are dependents of military personnel specified in subparagraph "a" of paragraph 1 of this Procedure, and living together with them - a certificate of the established form, issued by the personnel authority at the place of service of the military personnel, confirming that this person is dependent on the military personnel, as well as a document confirming cohabitation of a dependent with a military man (passport - copies of pages 2, 3, 5 - 12 (registration at the place of residence) and 16 - 17 (children); disabled since childhood - conclusion (certificate) of a medical and social examination on the establishment of the appropriate disability group, and also a sanatorium card in the form N 072 / y-04 28 (for children - a sanatorium card in the form N 076 / y-04 29) or a health certificate when sent to a rest home, recreation center, children under 14 years old - analysis for enterobiosis, conclusion of a dermatologist about the absence of contagious skin diseases, a certificate from a pediatrician or epidemiologist about the absence of contact of the child with infectious patients by m place of residence, in a kindergarten or school.

If a family member of a serviceman or a pensioner of the Ministry of Defense arrived at a sanatorium-resort institution together with a serviceman or a pensioner of the Ministry of Defense and is entered in his identity card (passport), then a certificate of kinship is not required. In this case, the sanatorium-resort institution in the voucher of a family member makes an entry: "Arrived (a) together with his wife (husband) or father (mother)" and indicates the military rank, surname, name, patronymic and number of the voucher of a military man (pensioner of the Ministry of Defense) ;

D) military personnel undergoing military service under a contract in formations and military units of constant readiness in positions to be recruited by soldiers, sailors, sergeants and foremen, and who entered military service under a contract after January 1, 2004 - a military ID, passport (if available), holiday ticket, health resort card in the form N 072 / y-04 30;

E) conscripted military personnel, cadets of military educational institutions of vocational education - military ID, vacation ticket, food certificate, health resort card in the form N 072 / y-04 30;

E) patients transferred to sanatoriums to continue hospital treatment - a passport (identity card or military ID), the conclusion of the military medical commission of the hospital, an extract from the medical history and a food certificate (for conscripted military personnel);

G) spouses and parents of military personnel who died (missing) while performing tasks in the North Caucasus region of the Russian Federation in connection with the performance of official duties after August 1, 1999, as well as spouses and parents of military personnel who died in the line of military service on the Kursk nuclear submarine - a passport, a certificate certifying the relationship to the deceased serviceman, issued by the military commissariat according to the recommended model in accordance with Appendix N 4 to this Procedure, and a sanatorium card in the form N 072 / u-04 30 or a certificate on the state of health when sent to a rest home, a policy of compulsory medical insurance;

H) family members of military personnel who have lost their breadwinner, parents who have reached retirement age, and disabled parents of senior and senior officers who died (deceased) during their military service, as well as senior and senior officers who died (deceased) after being discharged from the military service upon reaching the age limit for military service, health status or in connection with organizational and staffing events that had a total duration of military service of 20 years or more - a passport (children under 14 years old - a birth certificate), a certificate certifying a family relationship to a serviceman who died (deceased) during military service under a contract, issued by the military commissariat according to the recommended model in accordance with Appendix N 5 to this Procedure, a sanatorium-resort card in the form N 072 / y-04 31 (for children - a sanatorium-resort card in the form N 076 / y-04 32) or a certificate of health when sent to a rest home, recreation center, compulsory copper policy qing insurance. In addition, parents - a pension certificate (certificate of a medical and social examination on the establishment of disability), a certificate issued by the military commissariat stating that they were dependent on a military man, according to the recommended model in accordance with Appendix No. 6 to this Procedure, children under 14 years old - analysis for enterobiasis, conclusion of a dermatologist about the absence of contagious skin diseases, a certificate from a pediatrician or epidemiologist about the absence of contact of the child with infectious patients at the place of residence, in kindergarten or school;

I) persons of civilian personnel of the Armed Forces - a passport, a certificate of employment confirming their work in a military command body, military unit, institution (organization) of the Armed Forces, indicating the source of financing of labor costs (from the federal budget allocated to the Ministry defense, or income of institutions (organizations) of the Armed Forces received from entrepreneurial and other income-generating activities) according to the recommended sample in accordance with Appendix N 7 to this Procedure, a sanatorium-and-spa card in the form N 072 / y-04 31 or a health certificate with direction to a rest home, recreation center, compulsory medical insurance policy.

21. Citizens are admitted to sanatorium-and-spa institutions and discharged from them within the time specified in the vouchers.

22. When discharged from a sanatorium, a citizen is issued a discharge summary and a voucher for a voucher, and from a rest home and recreation center - a voucher for a voucher.

For citizens who arrived with food certificates, the date of their exclusion from allowance is indicated based on the actual time spent in the sanatorium.

A) recall from vacation in the prescribed manner;

B) sudden serious illness (death) of family members and close relatives: wife, husband, children, father, mother, brothers, sisters;

C) a natural disaster that has befallen a family or close relative;

D) transfer for health reasons to a medical institution (hospital).

In these cases, the persons specified in paragraph 1 of this Procedure do not lose the right to social guarantees for preferential sanatorium treatment and recreation in sanatorium and resort institutions in the current year.

V. Provision of vouchers to sanatorium-and-spa institutions for the continuation of hospital treatment

29. In order to continue hospital treatment in accordance with the conclusion of the hospital military medical commission, citizens from the list of persons specified in paragraph 2 of this Procedure are sent to sanatorium-resort institutions.

With a positive consideration of the application for a ticket (in the section for special notes), the sanatorium-resort institution makes an entry: "________________, (surname, name, patronymic) free of charge, to continue hospital treatment" and is certified in the prescribed manner.

32. In the absence of sanatorium-resort institutions of the required profile in the Ministry of Defense, sanatorium-resort treatment of citizens from the list of persons specified in paragraph 2 of this Procedure is carried out on a contractual basis in specialized sanatorium-resort institutions of other federal executive bodies or other organizations that have the appropriate medical license.

Vouchers for aftercare of the citizens specified in this paragraph, immediately after inpatient treatment are provided free of charge at the expense of compulsory social insurance.

d) (b) members of the families of military personnel and officers dismissed from military service upon reaching the age limit for military service, for health reasons or in connection with organizational and staffing measures, whose total duration of military service in a preferential calculation is 20 years or more, and with a total duration of military service of 25 years or more, regardless of the reason for dismissal (spouse (husband) and children under the age of 18, as well as older than this age, who became disabled before they reached the age of 18, children under the age of 23, studying in educational institutions full-time), as well as persons who are dependents of military personnel discharged from military service upon reaching the age limit for military service, health status or in connection with organizational and staffing activities, the total duration of military service of which is in preferential calculation is 20 years or more, and with a total duration of military service of 2 5 years or more, regardless of the reason for dismissal, and living together with them. At the same time, children under 18 years of age are sent, if there are medical indications, to children's sanatoriums. Children under the age of 18 who do not need special treatment are sent to sanatoriums and resorts where family vacations are provided, on vouchers together with their parents (parent);

A new procedure for sanatorium and resort provision in the Armed Forces of the Russian Federation has been approved.

The main changes are as follows.

With a total length of service of 25 years or more, the basis for dismissal does not matter.

Citizens are sent to the appropriate institutions by decision of the named Department. Before, sanatorium-selection commissions were formed.

It is fixed how citizens should be informed about the procedure for sanatorium and resort provision.

It is envisaged that a register of persons sent to sanatorium-and-spa institutions is maintained.
